CALLING INSURANCE FOR TESTING

  1. Look at the back of your insurance card.

  2. Call the Member Services number.

  3. Follow the prompts to check eligibility and benefits. You will need:

    1. Insurance holder member ID Number.

    2. Client’s (whomever is being tested) date of birth.

    3. Client’s first and last name.

  4. Determine your In-Network deductible, out of pocket, coinsurance, and copay and how much you have met towards each. 

  5. Check if your plan covers psychological testing; the following information will help.**

    1. CPT codes: 96132, 96133, 96136, and 96137 (these are codes billed for testing services).

    2. Place of Service: Office (Code 11; this is where the testing will occur).

    3. Provider type is Professional.

    4. Provider NPI is 1548885726

    5. Provider Tax ID is 83-3096410

    6. Provider name is ILLUMII Pathfinding Group 

    7. If you need a diagnosis code, use: F41.1 

  6. Determine if you need preauthorization for testing.** 

    1. If you need preauthorization for testing, you will need to get the phone number and/or website of where the forms can be located. 

            **You may need to talk to a customer service representative.

Please note: Insurance does not typically cover the educational portions of testing. You may be able to use your FSA/HSA benefits for this part of the cost which typically ranges from $1000 to $1250.
